Retatrutide in Spain: arrival timeline, TRIUMPH trials and how it differs from tirzepatide

While semaglutide dominates clinical practice and tirzepatide owns the headlines, Eli Lilly is writing the next chapter with retatrutide — a triple GLP-1/GIP/glucagon agonist that already crossed -22 % body-weight loss in phase II. Review of the regulatory status in Spain, expected EMA timeline, current grey market and how it differs clinically from its predecessors.

Retatrutide in Spain: arrival timeline, TRIUMPH trials and how it differs from tirzepatide

Regulatory status in Spain (February 2026)

Retatrutide (LY3437943) is NOT approved by the EMA nor authorized by AEMPS as a prescription drug in Spain. It is not listed in the official drug registry (CIMA) and cannot be dispensed at community pharmacies. Clinical use is restricted to patients enrolled in active TRIUMPH trials (Spain is not in TRIUMPH-1/2 per the EU CTR registry; centers in Madrid and Barcelona are active in TRIUMPH-3 under direct Eli Lilly funding).

Expected EMA approval timeline

Eli Lilly announced on its Q3 2025 earnings call that it plans to submit the BLA dossier to FDA and CHMP in H1 2026, with a possible EMA decision toward the end of 2027 or early 2028. The company guided analysts to expect a European launch in fiscal year 2028. That puts Spain (via CIMA + SNS reimbursement) realistically at 2028-2029 for public health access, with earlier private use at authorized clinics potentially available sooner.

TRIUMPH trials: what we know so far

TRIUMPH-2 (obesity without diabetes, phase III, N=2,300) read out preliminary data in September 2025: -24.2 % body weight at 88 weeks with the 12 mg weekly dose — the largest absolute effect reported for any incretin-agonist monotherapy. TRIUMPH-4 (in type 2 diabetes) reported HbA1c -2.1 percentage points and -16.8 % weight. Safety signal is comparable to tirzepatide: nausea and transient gastroparesis as the most frequent events, no new pancreatitis or thyroid signals.

Key difference vs semaglutide and tirzepatide

While semaglutide acts on a single receptor (GLP-1) and tirzepatide on two (GLP-1 + GIP), retatrutide adds a third arm: the glucagon receptor. This activates thermogenesis and energy expenditure — it doesn't just suppress appetite, it also increases basal caloric burn. Practically, phase II data suggest retatrutide may break the typical weight plateau of dual agonists around month 12-14, sustaining loss without aggressive dietary tuning.

Current grey market in Spain

Given the regulatory void, Spanish retatrutide trade has moved to the research-use-only (RUO) channel: independent labs offer the molecule synthesized in China and India at prices between €80 and €150 per 10 mg vial. Labels clearly read 'for research purposes only, not for human consumption.' AEMPS does not regulate RUO imports as long as the declared destination is strictly scientific — but diverting it to human use is illegal and carries administrative sanctions. Serious suppliers include HPLC CoA ≥98 % per batch and documented traceability.

What's coming in 2026-2027

Key milestone to watch: definitive readout of TRIUMPH-1 (obesity with moderate-severe sleep apnea, expected Q3 2026) and TRIUMPH-5 (HFpEF heart failure, phase IIb expected H1 2027). If retatrutide proves cardiovascular efficacy beyond weight loss — as semaglutide did with SELECT — the EMA dossier will benefit from public-health priority and the approval date could be pulled forward 12-18 months.
